There "shouldn't" be any consequences, as long as you give adequate notice, the more time before check-in you give them, the more time they have to replace you. If you call the morning of check-in and say you're not coming, they might probably put a black mark next to your name.

I recently did this at Worldwide Clinical San Antonio and they said after the 3rd time it happens, i'll be put on probation for 2 months. So to answer your question, i think it depends on the facility or company.

I haven't seen any negative consequences. The 3 clinics near me have taken to advertising on craigslist, tv commercials and the like to find people. And they will even call people that have done studies there to ask if they will come in for a certain study. They will post over and over again on facebook about studies right up until the last minute. This tells me they have a hard time filling all the studies... and are desperate for people to screen. So why would they blacklist someone like that? I mean, unless you were rude or simply were a no show... I could see. But if you called and cancelled, I think you're fine.
