I am JALR, and have also been an employer and employee.
I understand your need for stability/the new job AND that they hired you to do a job that included contractual obligations. You only live 3 hours away. Many who do studies there live plane rides away.
We all signed contracts which are legally binding to all parties during the course of the study. If you do not meet the stipulations in the contract, the penalties are spelled out, and can legally be exercised.
Consider all perspectives, and that everyone must do what is best from their perspective, within those legal boundaries/available latitude everyone agreed to.
I imagine they have a statistical variable for anomalies built in, and that if you miss an OPV, it upsets the protocol and creates issues that can skew the overall study data.
I'd try to negotiate a win-win and maintain communication and goodwill. The docs are also on a schedule based upon the original agreement. Check your contract: is it possible for you to get paid (full or partial) if you make the OPV with the time variation? Ask if it is possible to reach a mutually agreeable compromise, such as rescheduling the OPV for asap before or after the original time/date, so they know you are trying (goodwill), and can still get the data they need with a time variation?
They have an obligation to address it based on the contract you signed. If you DO NOT do a completion visit, that further impacts the usefulness of your data and effects their ability to pay you and close you out of the study.
If it were me, I'd email your study coordinator directly to address the options in writing, explain, and offer your intention for cooperation/goodwill (ing the door to the same), so everyone is on the same page legally and otherwise.

I have feedback about CNS to offer, in the hopes of offering transparency to other potential participants. CNS has several locations in different states.
I'm thinking of screening at Spaulding (WI) next week and would appreciate any feedback as their Google reviews are quite polarized.
Would be great to have a thread for each so we can compare notes...

Just had a pre-screen conversation.
Many places are OK with concurrent non-dose follow-up overlaps with an expired half life.
This place allows NO overlaps at all PLUS the additional half-life time, so don't bother trying or you might as well light a match to your travel funds...

I've also seen someone create an adverse effect t get out early with full pay, but not all places offer that. In fact, the majority of the contracts I've seen "prorate" for the number of actual days, period. It prevents fraud, but also screws those who legitimately have an effect that deserves attention.

This happens at PPD as well. The vets do not report anything. I had heart palpitations and almost passed out after the first dose and the advice was: "smile and show up on time, or you won't be welcome here anymore." I came upon one woman throwing up, doubled over in pain. She begged me not to go get help or say anything or they would send her home w(with less pay) and ban her as they had someone else. She was a single Mom trying to keep the rent paid. Her friends kept the admins away and made sure the got to the check-ins and vitals on time looking ok enough not to raise any flags.
I've experienced this elsewhere too.
This is an important issue for several reasons: 1- The sponsor and CRO want the drug to get approved 2- Adverse effects slow the process or can prevent that, and I've literally had the techs in more than one place complain about the paperwork involved (despite that it's one of the reasons they get a regular paycheck and benefits.) 3- There's huge money invested in getting drugs approved; corporations are all about the bottom line 4- NOT reporting effects is dangerous for everyone; for us and the multitudes it will be prescribed to later 5- This is a significant ethical issue we all have to live with
The prevailing point of view (politically and otherwise) seems more about short term profit than long term well being, and it is defeating the purpose of the trials and has the potential to create significant harm to many.
I see this as not much different than other "whiste blower" issues; Things cannot change unless we say soing, and there is risk if we do, but if one person is willing to do it and others say "me too," it can change for the better.
Has anyone here contacted the IRB about a problem? If so, what was your experience and lasting impression?

I've screened for other studies after the above experience in Austin. My arm veins were visible and productive when I walked in there and every place I've screened at since has remarked about scarring interfering with normal straight sticks. Luckily an experienced phlebotomist knows how to work with lower arms and hands and subsequent venues used catheters for multiple sticks or I would no longer be able to get past a screen.
Interestingly, addiction sites seem to offer the most info on healing veins damaged by too many sticks, and the prognosis isn't great.
DO NOT let your veins get ruined by inexperienced study staff.

For the "jet-setters"...
Most airlines allow you to cancel within 24 hours with a full refund, just be aware it can take an instant to a couple of weeks for the credit to show up in your account.
Southwest Airlines is awesome for those of us who travel. There's not another airline I know of that caters to us so well. Their policies allowing changes with full credit (available right away) and free baggage have saved me over $1000 in the last year based on the unpredictable nature of this business. The seating has decent leg room and it's easy to snag an emergency row seat of you need the extra inches. They offer complementary beverages, and their sales are amazing too.
Spirit Airlines may be cheap, but you're paying for your bags, and everything else..including the seat itself. You're literally packed in like sardines with no leg, elbow, or even room to lean and take a nap, so it's really not the best choice for longer itineraries and inpatient studies where you're bringing a carry on or checked bag. There's also no wifi (even for a price) and you couldn't get a free glass of water if your life depended on it so bring cash if you want to stay hydrated or a bag of chips, to avoid paying at least $30 for an extra bag to bring your own. Between the lack of space and being unable to recline even slightly, there's also no rest for the weary on those bicoastal red-eyes or fly-in the night before your screen or check-in, so what you save in airfare you spend on lodging if you want to show up in top form.

Yes, you should be worried. Heavy scarring many never go away. They may be paying you, and it's YOUR body, so you have a right to request a skilled vampire. Don't let some newbie with a needle wreck your health, earning potential and leave you in pain, looking like a junkie.
